How to prevent balnk entries to database?

I am using an HTML form in wordpress by which we can save the data to database. i dont know whats wrong with that code whenever i submit or refresh my page the data added to the table automatically blank or duplicate anyone tell me how to prevent from this? My code is here

as per my concern you can edit your database and set a column as primary key and set allow Null= false
