How to create a popup window with controls in acumatica ERP

Dear Friends,
I am using Acumatica ERP version 20.110 and Visual studio 2017.
I want to get a popup screen when click on a menu .Popup screen contains 2 controls say check box and texbox. Based on the data of check box and textbox, a new page to be opened and also need to pass data entered by the user.
