I believe that sometimes it’s necessary to reject a bug fix and ask its author to re-do the patch in order to protect the project from bigger problems. "You didn't say, please" didn't make the list?

Those were OK but sort of stupid reasons, IMO.
Here's my list:
- It doesn't fix the bug
- It introduces other bugs
- You monkey-patched the code instead of fixing the real issue
- You didn't use your brain -- ie, there are wider, fundamental flaws in the business logic
3 is the most commonly used form in duck-typed languages like Ruby, but I've seen it done (and sadly, done it myself) in typed languages as well.
4 I find often enough, again in my code as well, and the temptation to fix the narrow problem rather than say, guys, we need to rethink this whole piece here, is hard to overcome.

A few valid reasons to reject a site from publication in the insider.
1) An obnoxious cover almost the entire screen ad when the page was loaded.
2) Publishing content copied from a second site[^] that doesn't do that.
3) I haven't had a good excuse to dis any of the other sites you've used recently. 
Did you ever see history portrayed as an old man with a wise brow and pulseless heart, waging all things in the balance of reason?
Is not rather the genius of history like an eternal, imploring maiden, full of fire, with a burning heart and flaming soul, humanly warm and humanly beautiful?

Yeah, seems like everyone is moving to these popup style ads. But this one is particularly bad. OK, skipping them from now on.

In central Ohio, recruited grads with credentials can expect 60k -> 70k at larger companies like JPMorgan or Cardinal Health, but smaller companies in the 45k -> 60k range.
