For the purpose of seperating my accounts to whatever's needed, I have the following accounts:
1) Personal Account - for Social Networking, and personal stuff (oft regarding personal life), and development forums like CodeProject.
2) Gaming Account - for Gaming News, a Gaming Twitter and for whatever gaming portals (like Steam, NNID, PSN) I feel like.
3) Facebook account - Just for Facebook. It's both a security and an identity thing for me, since I don't use it so much.
4) RP account (Role Play account) - for whenever I'm doing RP on sites.
My work E-mail is only for Work-related stuff, so it should stay that way. Don't want my employers to know much about me, so I refrain from doing any policies that could let them know more about me.

So I use a unique random email address for each site, and then use Keepass to keep track of it all (with unique secure passwords for each site to boot)!

If I need something that requires email submission at work, I tend to use my work email as at that time I am using that service for my job. Everything else I do at home is my own matter and the company doesn't need to know what it is. Of course I would talk to them in person about it, but I like to keep my job and personal life separate. If the company would require that I bring my own laptop to work, I would make another account just for things related to the company.

Same here. Occasionally I'll find something useful enough that I switch it from work to personal, in case I get laid off or something, but at sign up I try to maintain a bright line between the two.

I tried to start using address per site (as I have my own domain) but
a) the pain of changing all the existing sites (if they even let you!)
b) not enough discipline.
c) not always easy to work out what to use, if its a game login for example.
Wish I'd stuck with it though.

I use a combination of fake and junk email addresses. I have a Yahoo account that I never look at which is used for websites.
I also often use "foo@bar.com" and "blowme@devnull.com". Every so often I a message stating that someone else is using that same email address.

..but I tell the co-workers that this is my work email address so they cannot send me emails bothering me on my Birthday. And for the family it is a personal email, so that they cannot look for any female co-worker sending me "I love you" email! 
And actually, I don't have any co-worker and I am not even married to have someone checking my emails!

My dear old pater, alive as he was in the days when spam meant spiced ham and unsolicited mail just made the postman grumpy, and was used to light the fire, resented the fact that companies, about whose services he would enquire through the cards included in many technical journals, would sell his details on to less scrupulous companies on whose mailing lists he would find himself.
So, whenever he filled in a card, he would use a unique name - usually using the same first initial then a different middle initial - then he would be able to track back unsolicied email to the original - at which point he would invoice them for selling his name without his consent.
He never got paid!
In the bad old days I did a similar thing. But now life is too short, I get very little spam anyway, and automatically filter out any that make it to my home email.

I've been working at the same software company since I graduated from college over 16 years ago and during that time my work email is the only email I've ever used. I've experienced varying waves of spam over the years, but nothing particularly terrible.

Yes, you probably are a little odd.
I have no problem with sticking with a company - it's a good idea if you and they get along and so forth.
But...if I wasn't self employed, I'd never use the same email address.
2 reasons:
1) Personal stuff is just that: I don't want it on company servers, because it may contain things I don't want the company to know: how drunk I got last Friday and what happened as a result for example. And I don't want work email at home: they aren't paying for my time then so I'm not dealing with work matters.
2) If you leave, get fired, or the company goes bust, what happens to your personal emails? The history, the contacts - you've got to contact all of them and move to a new address.
Much better to have two: separate work and personal so you only get the ones you want.

BTW, I went the extra step (that few people take) and don't rely on Gmail, Yahoo, MS, Hotmail, etc. I use my own mail server at my own domain.

I have about 30 email addresses. If I am interested in whatever comes my way from a particular site (eg. banking, gas company etc.) I use an address that I check frequently. If it is somebody who is going to spam me (eg. concert tickets, one-time catalog purchase...) I use a YOLO address or one where I rarely check for messages.
Not a spam fan...
I'm retired. There's a nap for that...

I would have checked "Always use Personal email" if that had been available. Also the category "Usually Personal email, but sometimes Fake email" should have been offered. Just my $0.02.

I have a Windows Live email address I use when I expect I'm going to be spammed and / or don't trust the place.
Also use my Windows Live for my Windows 8.x login

The context of the site decides what email address I use. For work, use work address. For home financial stuff, one personal account. Something I'll need quick and easy access to, my personal gmail account so I can get it easily on my phone. For less reputable stuff I have a yahoo account.

My online activity follows my life-style (not vice versa).
* If I'm registering for work-related intention - work email. I don't want constructional spam or junk here.
* If I do not trust it or to receive constructional spam (mails that are not unwanted, but possibly either too many to read or interesting, but not important - like daily news, etc.) I use personal gmail.
* If I trust 'em or want to make an impression or when privacy is important - I use my personal local email (yeah, I have email server and I'm proud of it).

However, if I know that I'm just going to get spammed but for some reason or other I have to supply an email, I use outlook.com. It's where I direct all the junk mail.

Like Matt Tomlin (this thread), I use a catch-all (actually, one of several). Some mapped address fork the target address so as to allow me to see important mail sooner.
This allows each site an always available unique address and supplies the additional benefit, should I start receiving spam, as to know who sold me out.*
* There are methods to punish these people (spamming business and seller, alike).

alternative algorithm because :
- i use work email for work stuffs like plugins registrationg i need, visual studio stuffs
- i use personal email for other things that i trust
- i use my alternative email for sites that i don't trust
