you are assigning
CityCode = @FatherName,
Address = @Id
BirthDay = @MeliCode
FatherName = @Tel
ID = @Tel
CodeMeli = @Address
tel = @BirthDay

and @CityCombo is extra.

check the order and insert in proper order. then it will work.
