If you search around, you will find tons of images for timesheets on the web.

For example, take a look at:

This is a classic example of a "duration" based timesheet, where the employee just enters the number of hours he/she spent on a task; you don't know what time of day they performed those duties.

The other type of timesheet is where you list a task an the start and stop time of the task. You can then see when a person arrived, took a break, performed certain duties and left for the day. You can think of this as an automated "punch clock" type of timesheet.
