Microsoft Outlook used to work fairly well at detecting and rejecting the trash, but lately it's failed completely to catch garbage, despite the rules I've established. Ten years ago or so I tried out a variety of spam blocking products; all of them were seriously limited in performance, and none were worth the trade-off. Has that changed? Is there a reliable, effective anti-spam product available today?

I think our situations are different, but I had the same problem about a month ago. Out of the blue, I started getting vast amounts of junk.
I run MS Exchange from home, and it turns out that it had something called ForeFront running alongside it which acts as a spam filter, but that was only a 90 day trial. End of trial and it turned off.
That product seemed very good at sorting it all out, the caveats being you need to run Exchange, and also I think MS have abandoned it now. I'm looking for a solution to.
Actually, it would be a very easy thing to code up, if only you could plug your own spam filter in somehow.
