Please note that before inserting question to forum you must try at certain level then only we can suggest further

1. First create selection filters (suppose dropdowns etc...)
2. Based on these selected filters send parameters to procedures and set these values in dataset and set datasource and display in crystal report viwer.

Use Parameter-fields to filter(particular patient id) the data in report, check these
