I want to use a custom php extension library with a web app hosted by a commercial hosting server. Since I don't have admin access to the server, is there still a way I can get to still use it on the server? Surely, people must have been using custom php extension on commercial hosting servers before now.

If this would be possible, all websites would be hacked in one day :-)
If it's a real PHP extension library, you have to contact your hoster and ask if he is willing to install it (because most hosters will run several websites in a single machine, this will only be possible if you are paying for an own machine or virtual machine).
If your PHP is a simple script, place it directly within your domain and call it from your webiste.
