How to do what? If you want a bad design, create a small form, add 50 checkboxes and a textbox to it. Or, create a user control, add a CheckedListBox control and a textbox.

Assuming that you have a copy of Visual Studio you just drag the components from the toolbox onto the form. However, I doubt that any user is going to be happy with such a bad design.
