i want to uload dynamics form on internet ex i want upload feedback form to my website so i done all insert and connectivity in that form but i have problem that how to upload that database table on internet

Why do you want to upload db table on internet? Your feedback form will be accessible over internet once you publish your site using web server.

hi you should use phpmyadmin create database and go to import tab then here you should have to upload your file. before that you should have get back from your current database.
