I would say that if the app you are working on calls for "simple maintainable code" then yes, what you list is just fine, but typically what I have seen is that apps dictate the architecture - or at least they should!
If an app needs maintainability and features that layering provides then yes, use layers. If it doesn't and needs to be fast as hell, or meet some other requirement that layers does not provide, then don't use layers. I guess my point is that the apps I have seen don't always have the same requirements and therefore need to be architectured differently to meet those requirements. My 0.02
