Agile promises continuous improvement through an iterative process, but without proper checks, project failure is commonplace Whodathunkit?
"Projects fail due to lack of planning". Yup. Agile or no.

The other dead-on observation, which I've seen: "...the iterative process may lead to iteratively improving, one failure at a time, towards the wrong outcome."
I've yet to see a genuine agile project actually work. The ones that seem to have worked have so much planning and upfront design that it really wasn't agile, even if they used all the words and pretended otherwise.
