This appears to be a caching issue. We have been investigating this, but due to its very intermittent nature is hard to pin down.
Fortunately, the cache is refreshed often to keep the data fresh, so it fixes itself quickly.

I think that the closure of question Is there any way to use vertical bar - | - between variables?[^] could be reconsidered.
I agree that the question seems to pose a desired solution, but it is not the case that it "has not been phrased in a way that allows a reasonable answer to be provided", since I provided a solution which I think was quite reasonable.
Alternatively, if a question is closed that has solutions, then at least keep those solutions accessible to be read.

That has happened to me several times.
You should consider whether that specific type of report should be available for questions that have actual answers.

An answered question should not be deleted anyway.
First: The one who answered spend some effort (hopefully), and at least than it should be not the choice of some member's mood to destroy this.
Second:
If an answered question can not be deleted, it will show the Praxis of some experts here, who answer and same time Report the question.
Only my view....

It could be bypassed as well: You can delete the answer first and then the question.
I see your point, but there will be always a way to bypass. If not... we can be eaten by spammers and other abusers.

Yes, there is unfortunately a very easy way to Bypass. I tested it with a fake account: I answered a question and immediately deleted my answer. Doing this 10 time (+-), my fake acc appeared in "top experts"

This is possibly an issue with the article editor.
This[^] tip (pending approval) has a broken image, but when previewed in the article editor, it looks fine.

The is exactly what happens when you try to edit an article and the WYSIWYG comes into action. The tip is good in currently live version, but I cannot see the tip in moderation, did you pull it out? The problem is still the Article ID being prepended to the image-name. Anyways, read this article as it covers why do this bug appear and how to solve it.
