Its might be you have not used CSS and HTMl in proper manner .. its not asp.net .. finally things rendered in html code dear .. so better use proper CSS .

Why not .. use user control instead of master page .. or you can just copy and paste the same ui code again and again .. but it dont seems good coding practice ..Don't repeat yourself . :) :)

while installing your application ..one way you can do this when your application
installed and when it runs first time you can create on first form load .

check that unit where you published your code , which IIS version is there and which framework on that unit .if a code is working on your unit , then check the IIS and seeting where you published the code.

Don't get too tensed and panic,first explain your query clearly and this is not the place where people will code for you .people over here to help you from some problem not to complete your task .
and where i m trying to understand your query , your task cam be completed easliy using SSRS.
