This is one of those scenarios where I really have to question the ethics of what we do in the software/IT industry. Shouldn't someone tell the police they can just mount the hard drive as read only? Actually shouldn't they have IT specialized forensics investigators who know that?

Citibank was acting as an agent for Revlon, which owed hundreds of millions of dollars to various creditors. On August 11, Citibank was supposed to send out interest payments totaling $7.8 million to these creditors.
However, Revlon was in the process of refinancing its debt—paying off a few creditors while rolling the rest of its debt into a new loan. And this, combined with the confusing interface of financial software called Flexcube, led the bank to accidentally pay back the principal on the entire loan—most of which wasn't due until 2023.

It looks like Flexcube is Oracle Software.
While Microsoft are not brilliant in some of their UI design, my experience with pretty much anything Oracle UI is that it's as if they want to make things difficult for the user.

Ha! happened to someone else I see. I wrote code, shoot 20 years ago that misbilled customers (the tech lead was supposed to check this). Accounting found a mismatch in what the system was doing and what it was billing. Turns out I had an error in a transaction pre-amable. Since the machine didn't know what to do with the transaction, it parked it and emailed the trouble account.
The only problem was no one was checking the email.... had something like 45k emails asking for help.
The ugliest part was trying to explain to the customer why we needed to collect another $200K. Awkward...
